Reasons for Breast Revision Surgery

These days my patients are quite informed about the procedures they are requesting from me. They’re reliant on my professional advice on the details and implementation of their goals, but most Breast Augmentation patients have an idea about what the procedure entails. The lesser known sister of Breast Augmentation is Breast Revision surgery. There are a variety of reasons someone might need this “surgery after the surgery,” and I’ll cover those here.

Breast Revision surgery is for someone who’s already had a Breast Augmentation and needs a change that requires another surgery to the breast area at any point in time near or distant from the original Breast Augmentation. The majority of women love their new breasts with no complications and often experience satisfaction for many years.

However, a Breast Revision surgery is a great option for a variety of reasons, such as:

  1. Address the natural effects of aging and gravity on the breasts. The breasts with the implant still will sag over time and so a Breast Lift could help restore perkiness and placement.
  2. Breasts change as a result of pregnancy, weight loss or gain. If the Breast Augmentation occurred before these events, a Revision can be intended to restore your breasts to their former stature.
  3. Change the implant size with a Breast Implant Exchange to go smaller or larger. Fortunately, the Breast Implant Exchange is less intense of a surgery than the original augmentation. The space or pocket for the implant has already been created so there’s not the stretching discomfort experienced during the initial surgery. Also, the recovery time is usually shorter.
  4. Repair breast implant malposition. The implant didn’t settle where it should be and has caused the breasts to look unnatural.
  5. Correct capsular contracture where the tissue surrounding the implant contracts and squeezes the implant into an awkward and uncomfortable position. In this case, the constricting tissue would be removed and the implant replaced.
  6. Change the implant type. Some women who got saline implants and experience rippling or wrinkling, prefer to exchange them for the consistency of a silicone gel. This is particularly common for women who got a Breast Augmentation when the silicone option was off the market from 1993 to 2006.
  7. Breast Implant Exchange for prevention or in the case of leakage. Implants do not have a lifetime guarantee to not leak. It is recommended to exchange for a new implant every 10 years. If a saline implant is leaking, it’s easy to detect because the breast will appear deflated. However, in most cases, a silicone implant leak would require an MRI to detect leakage, so regular checks are recommended.
  8. Breast cancer treatments may require the implant to be removed. This is a decision that should be made along with your medical support team.

All About The Breasts: Surprising Facts, part 2

The more you learn about your breasts the better you can care for them and appreciate them. Here are some more fun facts to help you know more about your girls.

Side sleeping is best for breast shape. They even could use some pillow support while you sleep. This is just another reason that belly sleeping isn’t best for your body.

Breasts can be a breeding ground for bacteria. This happens because sweat can collect and the abundant oil glands above and between your breasts to make the area vulnerable to clogged pores and pimples. Prevent problems by wiping the area right after workouts or if you’ve been outside sweating a lot, and never staying long in sweaty bras because they can harbor bacteria that love dark, damp places.

They have growth potential through your early to mid 20s. This is especially important for younger women when considering breast augmentation. Though most breast growth does happen through the puberty years, they do continue to grow into early adulthood. How big they do get is actually determined by genetics from both parents. After the mid 20s, it’s unlikely for them to grow unless you gain weight, get pregnant, breastfeed (however they will go back down afterwards) or sometimes with birth control.

Nipples usually get darker during pregnancy. The body does this because it is anticipating breastfeeding the baby, which can better see darker colors. The darker nipple makes it a more visible target. No reason to freak out though, most women’s nipples lighten back to normal a little while after baby is born, while some may stay just slightly darker.

The creation of cleavage isn’t just based on breast size. While the larger ladies do have a cleavage creating advantage, breast shape and location on the torso also come into play. This means that the same cup size can vary in their ability to create cleavage. If the breasts are fuller in the outer breast or positioned wider apart, cleavage may not come as readily. A narrow torso, breasts set closer together or fuller in the center have an easier time creating cleavage.

Exercise can’t increase breast size, but the right ones can help their appearance. Breasts aren’t made of muscle, but they’re resting on top of muscle. These exercises can help firm up the pectoral muscles, which can slightly perk up the breasts: pushups, bench presses, dumb bell flies, medicine ball slams or pullovers. If this doesn’t provide enough of the lift and firmness you’re looking for, a Breast Lift can definitely enhance the positioning and perkiness of the breast.

Plan Your Procedures

While you may initially come into my office to address an immediate beauty need, we will take the time in our consultation to talk about your long term beauty goals and to help you with a plan. This plan may start off with some non invasive treatments, such as a Liquid Facelift using a variety of injectables to touch up and rejuvenate some crows feet, furrowed brows or mouth parenthesis creases. Fall

After the activity of summer and before the flurry of winter festivities, the fall season offers a perfect break in the social calendar for most people. This means that recovery time away is easy to schedule. Also, the cooler weather means clothes with more coverage and comfort. After getting any kids and grandkids settled into school, the scheduled routine or empty nest would allow for effortless predictability and rest for scheduling a surgery. It also will allow plenty of healing time for you to approach the holidays recovered and looking great. This is a good time for anything from Liposuction, Blepharoplasty, Facelift to Breast Augmentation. Fall is my first choice for cosmetic procedures.

Winter

This season offers some good opportunities for cosmetic surgeries. Before the new year, the injectables are the best because you can’t afford downtime between Thanksgiving and New Years, but January is the perfect month to kick off the New Year with a big step towards your beauty and healthy body goals. Winter scarves and turtlenecks easily cover the bruising or swelling from Kybella or a Facelift. Also the bulkier clothes will conceal any over night changes to the body from a Tummy Tuck or Liposuction. Spring

Choose Your Breast Augmentation Surgeon Carefully

You can do all the research on the best implant, placement and incision for your breast augmentation goals, but if you don’t have a board certified surgeon with experience and artistry, then your results likely won’t be what you’d hoped for. This is why it is so important to choose your surgeon carefully. What sets a surgeon above the rest?

Credible Education and Board Certified
The surgeon must have the appropriate training and certification. Board Certification is a must. Breast Augmentation General Q’s & A’s

Breast Augmentation General Q & A

Even though Breast Augmentation is the most popular cosmetic surgery procedure, there are a lot of questions women have.This Q & A is an overview of some of them… but my intention here is not to replace the consultation. The consultation is an invaluable part of my process. It is crucial to get to know my patients personally and to get on the same page. What Is A Breast Augmentation?
Breast Augmentation specifically addresses increasing breast size and shape. Other surgical procedures related to the breast are:
Breast Reduction makes large breast smaller and more manageable. Patients with too large of breasts suffer from back pain and other issues, and a reduction can provide great relief.
Breast Lift helps sagging breasts look younger and perky. This is great in combination with an Augmentation or Reduction.
Nipple Reduction specifically addresses only the nipples, which can get stretched out during breastfeeding or may be out of proportion in relation to the overall breast size. A Nipple Reduction can bring the nipple size in balance with the breast for a more aesthetic appearance.

Saline or Silicone?
Saline is a silicone shell filled with sterile saline, or salt water. Silicone is a completely silicone implant. Both are shown to be safe, effective and dramatically improved over the last few years. Within these categories, there are many kinds of implants to consider. This decision is highly up to personal preference and goals. As there are quite a few aspects of this question to address, I will dedicate my next blog just to this topic.

What Does The Surgery Involve?

  1.  Preparation: Consultation and decision on the size and type of implant, location of implant and incision location. You may need to get a blood test, stop smoking, adjust medications and vitamins.
  2. Anesthesia: General anesthesia in our highest quality outpatient center will help you feel completely comfortable during surgery.
  3. Incision: Your body type, breast size, type of implant, and scar preferences will help us to determine the optimal location (under breast line, armpit, belly button, or areola).
  4. Insertion: Many factors will help us decide whether the implant should be inserted into a pocket above or below the pectoral muscle, including the amount of breast tissue already present, drooping, position and size desired.
  5. Closing Incisions: This will involve internal sutures within layers of the breast tissue and on the surface skin closure.
  6. Recovery: Like with any major surgery, there will be a period of discomfort that can be managed by oral pain medication. The first week is low key while wearing a support bra and possibly tending to a drainage tube for a few of those days. After 2 weeks normal activities, except for exercise or strenuous exertion, can resume. We will see you and check on you to make sure you’re doing ok. It’ll take between 4 to 6 weeks to feel somewhat normal. The skin can continue to feel tight and sensitive beyond that, but it shouldn’t impede your everyday life.
  7. Results: Over and over again, my patients have confirmed that the recovery is worth the results. The implants take a few weeks to “settle” into their position and the scars will take some time to fade, but your body satisfaction and restored femininity are long lasting and well worth your investment in yourself!

Breast Augmentation with Fat Grafting

As an expert in Fat Grafting (also known as Fat Transfer), I’ve seen how wonderful this completely native material…from your own body can naturally enhance features without the use of foreign substances. It does require great skill to be used correctly and does have some limitations. But for my natural minded patients, this could be a great option for facial volume loss and the Brazilian Butt Lift, as I’ve previously written about. Depending on my patient’s expectations it can even be an option for breast augmentation.

Managing Expectations of Fat Grafting
I like to have an extensive discussion of my patient’s expectations and their options with any procedure, but when it comes to breast augmentation, there are quite a few options to discuss. These discussions include what size breast would be harmonious for the body, the kind of implant, the type of incision used to insert the implant or whether to even use an implant. Often, fat grafting isn’t considered by my patients because it isn’t as widely publicized as implants, but I do believe that it can be a great option for many.Here are some of the reasons to consider Fat Grafting for your Breast Augmentation:
… It’s a completely natural substance coming from your own body.
… Because it’s from your own body, there is less chance of your body rejecting it.
…The results are as natural looking as you can get.
…There’s no risk of silicone leakage or integration into the body, so no follow ups years later and no worries.
…”Upkeep” surgeries aren’t necessary.
…No scalpel is required, as the fat is extracted and relocated to the breast with a needle.
…A minimal 2 to 4 day recovery time is needed, compared to weeks of recovery after a breast augmentation with implants.
…The fat used for the breast is taken from an area of the body that can spare some, so while you’re enhancing the breasts, you’re getting the benefits of liposuction elsewhere in the body.

Fat Grafting is a Less Invasive Option

When done by an expert in fat grafting, like myself, it can add natural looking volume to the breast without lumps, bumps, clumps or clusters. It also encourages a natural response, from the body, that makes it long lasting. No drainage ports, compression bands, or weeks of recovery while waiting for the pain to subside and the implants to “settle” into their place. A breast augmentation with fat grafting looks truly natural… after only a few days, when the swelling and bruising from the injections subside. And no worries if you’re needle phobic. You can remain totally comfortable while sedated under general anesthesia, as the fat is extracted from another area and injected into specific places in the breast.

Natural is Not for Everyone
While fat grafting can create lovely results, it does have limitations. It may not be able to accomplish the increase in volume or shape that some women are looking for, which is why there is a large market for implants. There are many safe options with implants that offer a beautiful and satisfying breast augmentation.

AVOID FAKE LOOKING BREASTS…

This is the time of year women line up to get breast augmentations in order to look great for beach season or… to restore a more youthful look lost while aging, or to get back a pre baby body…and for many other reasons.  One common concern always prevails and that is the desire for the most natural looking results possible.

IMPLANT LINGO…Avoiding a “fake” look is a top concern today because many women innocently have breast procedures done by doctors who are not always qualified to perform them, or who lack the knowledge base needed to guide patients into the right procedures using the right implants or they have little to no experience, and also by others who simply can’t get the job done right for various other reasons. Board Certification and specialized procedural training helps to avoid these sad situations. Here’s the “lingo” or critical elements in breast augmentations:

  • SHAPE…implants come in round or teardrop shapes.  Round implants create more cleavage and fullness, while teardrop or stable form implants are shaped like natural breasts and they slope downward.
  • PROJECTION/PROFILE…Projection is the distance the implant protrudes outward from the chest.  Wider chests require minimal projection while moderate projection is best suited to narrow bodies and maximum projection offers the most fullness.
  • TEXTURE…Textured implants are firmer than smooth ones and may not look or feel as natural depending upon patient preference.
  • VOLUME…is how full an implant is. A natural look, using implants, may involve compromise on size.  Too much volume has different effects on different people.
  • PLACEMENT…Implants can be placed over or under the chest muscle.  Each placement has it’s virtues and should be well discussed with your surgeon before deciding on placement.

YOUNGER vs MATURE PATIENTS...Younger people today want more volume and “bigger sized” breasts while the more mature population wants to look “proportionate” but filled out (not larger) to overcome the ravages of aging.  But they all want this one thing:  an attractive size and pretty shape and position without looking “fake.” Nobody wants to look like they have “implants.”  The right surgeon helping you make the right choices at the right times can avoid the dead giveaways so common with fake looking implant jobs.

NATURAL LOOKING vs FAKE LOOKING AUGMENTATIONS:  Some breast augmentations are so good that you can’t tell they’re not the breasts you were born with (natural) while others are obvious from far off, and you know these are not natural.  There are 10 giveaways that tip people off to real vs fake breasts.  They are:

  1. Breasts are too close together.  Real breasts do not touch each other all the time unless you’re wearing a bra or tight top.  Natural breasts have a normal space between them.
  2. Breasts sit too high on the chest.  The top of the breast should start at the armpit. If they sit higher than this…they are probably fake.  High profile implants have a more pronounced look than their moderate counterparts or low profile implants.
  3. Breasts are too round…are a dead giveaway! Breasts should more resemble pears than round watermelons.
  4. NO natural slope…are a sign of large, round implants. Natural breasts always slope downward.
  5. GAP that keeps the breasts too far apart.  This indicates implants that are too large or are placed in a pocket that is too small causing the area between the breasts to look wide and bony.

Celebrities, Careers, Body Sculpting and the Lipstick Effect

Looking Good on Facebook Spikes a Rise in Plastic Surgery Requests:  Social media is about seeing and being seen. A survey of 752 surgeons revealed a 31% increase in plastic surgery requests from people wanting to look better on Social Media.  Procedures most requested were:  Botox, Rhinoplasty, FaceLifts, ArmLifts (Brachiaplasty) and Breast Augmentation.

Last Minute Beauty Fixes and Healthy Tips for 2013 Holidays

Christmas is just 9 days from today and I thought we could all use some reminder tips on how to avoid overeating, under exercising and staying out of trouble. As with Thanksgiving, we tend to overeat, skip our workout sessions and end up with a few unwelcome pounds, a muddy complexion, often with breakouts, sluggishness and regrets that we lacked the self discipline to avoid these problems.   shutterstock_36660592

1.  Understand your metabolism.  Each person is different and will burn calories at a different rate.

2.  Avoid repeated high calorie meals…for example enjoy Christmas dinner and avoid ongoing eating of the heavy foods.

3.  Eat more living foods (raw fruits & vegetables) like salads.

4.  Eat a filling low calorie meal before heading into the kitchen to cook for the crowd.

5.  Diet before the big day and after it….never diet on party or family reunion days.

6.  Eat smaller portions and still satisfy hunger by chewing your food until it is “liquid.”
