MINI TREATMENTS ARE IN HIGH DEMAND, are more REFINED and are HERE TO STAY.  I’ve blogged about:  Mini FaceLift to “refresh the face”, Mini NeckLift to take years of aging off the lower 1/3rd of the face and Mini EyeLid Lifts (Blepharoplasty) to rejuvenate the eyes.  There are other “mini” procedures. The eyes are one of the first areas of the face where aging becomes highly visible.  The eye area has thin skin and fat deposits, that over time often get lost or displaced, while unwelcome dark circles, hollows and bags develop and become more visible.  This blog continues my short series on “mini procedures” what they are and why they’re here to stay.

MINI EYELID LIFTS are among the top procedures requested today. They are called by different names such as:  Pinch Lift, Skin Pinch, Mini Eyelid Lift or Skin Pinch Blepharoplasty. Each surgeon has usually developed special techniques for their procedure of choice.

I do full blown eyelid surgeries (Blepharoplasties) and Mini Blepharoplasty procedures.  My mini eye signature procedure is the “Skin/Pinch Blepharoplasty.  This is a conservative eyelid tuck involving skin only and is ideal for patients who desire more space for applying eye shadow as well as a more defined upper eyelid crease.  My Mini Skin/Pinch Blepharoplasty takes about 1 hour: involves some swelling and bruising that resolves by itself over 5 to 7 days, and General Anesthesia is that of choice for this surgery.

Mini Skin Pinch Blepharoplasty surgery does the following:

• Achieves a modest but natural refreshed look
• Increases space for applying eye shadow
• Beautifully, naturally defines the upper eyelid crease
• Involves the least risks possible
• Erases a tired look in the upper facial third
• Refreshes, rejuvenates, renews youthfulness to the eye area

The Limitations include:  bags and eye hollows are not addressed since the fat and muscle is not transposed or eliminated…only extra skin is removed with this mini procedure and it may need future maintenance as many mini’s do.

Here’s a comparison of the “Mini” and “Traditional” Eyelid Surgery:

MINI BLEPHAROPLASTY:  Best for those with good skin elasticity and minimal wrinkling on and around the eyes…without under eyelid bags.  This takes about an hour, healing about 1 week, little to no bruising and less swelling than with traditional. Results last 3 to 10 years.   TRADITIONAL BLEPHAROPLASTY:  Best for those with heavy under eye bags and loose facial skin in the eye area.  Takes from 1 to 3 hours and is one of the easiest procedures from which to recover. Swelling and bruising lasts up to 10 days at which time you are about healed.  Results typically last from 8 to 15 years.

ASIAN EYELIDS are unique and require special knowledge, training and skill.  Asian eyelids lack a crease, usually have extra skin or epicanthal web towards the side of the nose.  I have much experience and expertise in creating the ideal eyelid for those subtle needs.

Botox is not new.  In fact, it’s been used worldwide for decades for both functional and cosmetic treatments. Botox is currently approved in 85 countries for at least 25 medical treatments such as Bell’s Palsy, Blepharospasm, Elbow muscle stiffness, chronic migraine headaches and many more. Synkinesis is pain and discomfort caused by nerves that have regrown into the wrong facial areas thus causing misdirected muscle responses. This occurs during the recovery phases of Bell’s Palsy when the regenerating nerve fibers initially migrate through a common channel called the main nerve trunk.  These soon divide into branches bound for the brows, eyes, cheeks, lips, neck and these migrating fibers are forced to select a path.  Sometimes, some nerve fibers earmarked by the brain for a particular function, may ultimately choose the wrong path and grow in the wrong location.  When this happens, for example, fibers responsible for smiling may grow to the muscles at the corner of the mouth and eyelid thus causing lip movement and eye closure instead of a smile. These are abnormal, hyperactive functions called Synkinetic function. These responses include “tears” when laughing or chewing, abnormal blinking when talking or chewing and other discomforts. Botox injections have proven very effective in hosts of patients suffering with Bell’s Palsy and other neuromuscular disorders.

Asymmetry (facial) resulting from Bell’s Palsy is one of the top reasons patients seek my treatments.  Botox, when injected into isolated spastic muscles reduce the synkinesis symptoms allowing for retraining of the brain and muscles while the Botox blocks the nerve/muscle actions.  Physical Therapy is typically coupled with the Botox treatments for ultimate results and the ongoing retraining process. KELOIDS are a type of scar that results from an overgrowth of collagen type tissue at the site of a healed skin injury.  Keloids can be firm, rubbery lesions or shiny, fibrous nodules that vary in color….some are pink, others flesh colored or they may be red to dark brown in color.  Keloids are benign (not cancerous), are not contagious but often cause severe itching and changes in skin texture.  Severe keloids can affect movement of the skin.

Keloids are often confused with “hypertropic scars.”  They are not the same. Hypertropic scars are raised and they do NOT grow beyond the boundaries of the original wound.

Since I have specialized training and years of experience in surgery and reconstructive surgeries, I have dealt with so many patients in need of Keloid removal and scar revision…I couldn’t begin to number them.  Just know, that dealing with Keloids is serious business and you should see a surgeon with expertise in this area…in order to get desirable results.

KELOIDS…WHERE THEY OCCUR ON THE BODY...Keloids can develop anyplace where an abrasion (injury) has occurred.  They can even result from pimples, insect bites, scratching ones self, from burns or other skin trauma.  They can develop post surgery.  They most commonly occur:

• On the central chest
• The back & shoulders
• On Ear lobes
• On arms & over the collar bone after body piercings
• From shaving…razor bumps may become keloids
• Bottom of feet
• Can occur at any age…but generally don’t until past age 11
• Keloids…may be linked to hereditary tendencies

COMMON TREATMENTS FOR KELOID REMOVAL

1)  Corticosteroid injected directly into the tissue – typically 3-4 treatments spaced about 1 month apart – will reduce a raised Keloid to a flat scar and reduce redness, itching and burning.

2)  Surgical Removal is used for larger, more extensive formations.  Technique involved may employ a scalpel, electric knife or laser removal.

3) Post-Surgical Complimentary Treatments are often used to minimize Keloid recurrence after surgical removal.  These treatments include:   -silicone sheet dressings applied directly to the skin around the incision; -compression therapy the mechanically flatten the scar formation, – topical imiquimod or advanced injectables such as bleomycin,  -interferon or 5-fluorouracil.  These drugs work at the cellular level to disable the Keloid tissue formation process.

4)  Laser Excision followed by monthly intralesional steroid injections alone or in conjunction with imiquimod.

These are my treatment preferences…even though others may exist.  I stay with the tried and true…those options proven to get desirable results.

AGING, CHIN AUGMENTATIONS & LIP REJUVENATION My last 2 blogs covered Chin Augmentations and creating or restoring beautiful, full lips through Lip Rejuvenation (injectable fillers).
