Getting it right: REVISION RHINOPLASTY

 

Very few surgeons are truly qualified to perform the complicated procedure of Revision Rhinoplasty. Revision or Secondary Rhinoplasty is a correction, whether functional or aesthetic, to the unsatisfactory results of a previous Rhinoplasty (Nose) surgery.

WHEN TO GO FOR THE REVISION… Typically, the recommendation is to wait a year after the original Rhinoplasty so that there is ample time for healing and the nose to “settle” into its new anatomy. There can be a shorter duration to this recovery time depending on the extent of the previous surgery, which can be discussed during the consultation.

Although the decision should not be made lightly, you should consider a revision if you have any of these issues:

  • persistent breathing problems
  • nose still too large
  • still a bump in the bridge
  • new nose not in balance with the rest of the face
  • too much removed
  • tip of your nose needs better refinement
  • tip is deflected off to one side (“twisted tip”)

WHY IS REVISION RHINOPLASTY MORE COMPLICATED?

  • Changes to the typical anatomy and geography of the nose due to cartilage removal during the first surgery
  • There may not be enough cartilage in the nose to work with and it has to be borrowed from behind the ear, which also requires extra incisions
  • Navigating the scar tissue in the nasal lining from the primary surgery
  • There is a higher risk of developing little spider veins in the nose called telangectasias
  • All of these issues increase the variables and the amount of time in surgery

SURGICAL EXPERIENCE IS CRUCIAL… It is possible that the problem resulting from the original Rhinoplasty may not be the fault of the original surgeon. Anatomical variations and the healing process aren’t under their control. Although, the actual surgery is under the surgeon’s control, which is why you need the most experience surgical hands for the best possible outcome. This is especially true with navigating the altered anatomy and scar tissue present during a revision Rhinoplasty. It requires not only skill, but an artful eye, as well.

Breathe Easier with Rhinoplasty

Do you have trouble breathing through your nose? One of the possible causes is a deviated septum. This can happen as a result of genetics or an impact caused by an accident or athletic event. A deviated septum makes breathing difficult because the bone and cartilage that is supposed to be down the center of the nose that divides the nasal cavity in half is crooked or drastically off center.

  • The best way to confirm a deviated septum is to see a specialist like myself. The following are common symptoms of a deviated septum:
  • nasal congestion on both sides or one side being more congested than the other
  • difficulty or loud breathing
  • repeated sinus infections or constant postnasal drip
  • aching in the head or face
  • issues with breathing during sleep, such as snoring or sleep apnea
  • nosebleeds

The specific surgical procedure to fix a deviated septum is called a Septorhinoplasty.
TREATING A DEVIATED SEPTUM

Septoplasty is often combined with Rhinoplasty, or nose job, to correct the external effects of the crooked insides. The combination of these two procedures is called a Septorhinoplasty. Rhinoplasty is one of the most common surgical procedures and in the hands of a skilled surgeon like myself, the results can help you breath easier from both physiological and aesthetic improvement. Anyone in good overall health is a candidate, although for children needing septoplasty, it is preferable to wait until after the nose has stopped growing around age 15.

WHAT DOES SEPTORHINOPLASTY INVOLVE?
Septoplasty: By working through the inside of the nose, a small incision is made in the septum. The breathing canals of the nostrils are evened out by removing the excess bone or cartilage. Soft gauze or internal splints stabilize the nose as it heals.
Rhinoplasty: This portion of the procedure corrects any cosmetic imbalances in the bridge or nostrils to improve the nose’s appearance and overall harmony of the face. This recontour of the nose improves functionality and appearance to give optimal performance and confidence.
Both are performed on an outpatient basis and using either intravenous sedation or general anesthesia, depending on the patient’s health conditions and preferences.
RECOVERY… Oral pain medication will help mitigate the discomfort from either surgery. Septoplasty recovery should be within a week. Rhinoplasty recovery from bruising, swelling, and incision healing is longer than Septoplasty alone. It’ll last about a week or two, although some people can return to work after 1 week. Either way it’s recommended to rest of the head elevated to help with swelling. Even after the initial Rhinoplasty recovery of about 2 weeks, patients should refrain from intense exercise and contact sports for at least 1 to 2 months. This makes it imperative for athletes to correct their deviated septum in the “off season.”

COSMETIC and PLASTIC SURGERY TRENDS FOR 2015  Couples are having surgery together...

Some of the hottest procedures of 2014 will continue to grow this year, but there are some new predictions about what will be “in” for 2015. Last year buttock augmentation and neck rejuvenation showed some of the greatest increases. So what is in store for this year? Read on to learn about the trends for 2015.

Take Years Off Your Face with Rhinoplasty

Poor Rudolph the Reindeer. All he’s known for is his bright red nose. In reality, no person suffers from a bright red glowing nose, but sometimes those humps and bumps, that droopy or bulbous tip, or crooked bridge can make some people feel like they do. Ideally, the nose is meant to fit harmoniously with your unique facial composition.

RESHAPING WITH RHINOPLASTY… Commonly called a “nose job,” Rhinoplasty plastic surgery can enhance the nose in many ways. Cosmetics Mercury Poisoning, Medical Travel and Board Certification

shutterstock_91055561COSMETICS containing Mercury are poisonous.  A new FDA update begs your attention. Many face creams manufactured out of country and sold in the US, illegally contain mercury. Mercury causes serious health problems such as kidney and nervous system damage, poor brain development in the unborn and very young children, breathing/respiratory issues and much more.  Children are exposed simply by touching their mommies and putting fingers into their mouths or by breathing in vapors.  ONE EXAMPLE:  A 39 year old woman from California presented with 100 times more than the average amount of mercury in her urine and symptoms of mercury poisoning.  For three years, she and her husband had been using an unlabeled face cream brought in from Mexico.  Other immediate family members (one 4 year old), who did not use the cream, also had elevated levels of mercury.  Virginia, Maryland and New York have reported similar cases.  Mercury containing products have been found in 7 states so far.  Recent FDA investigations  revealed 35 products with unacceptable levels of mercury.

Products most likely to contain mercury:  skin lighteners, anti aging treatments, acne products, lotions and potions to remove age spots, freckles, blemishes and wrinkles. These are frequently sold online and in Latino, Asian, African or Middle Eastern communities.   Other names for mercury:  mercurous chloride, calomel, mercuric, mercurio or mercury.  Avoid them!  Stop using them if you are using them!

SAD TALE of TWO WOMEN!  Two women took a trip to Mexico in hopes of saving $$$$ on plastic surgery costs in the US.  They were excited about getting Tummy Tucks and Breast Augmentations until it was too late.  Both were excited about the discounts for plastic surgery in Tijuana.  They both knew it was too good to turn down.  The YouTube presentation filmed at Jerusalem Hospital in Tijuana was exciting. They went, they saw and they had surgeries.  The excitement turned to horror when the bandages came off and one of the women saw skin hanging out of her incisions.  Within two weeks both women required medical treatment and hospitalization…one was near death.  They have survived and want other women to take heed!  They ignored certain signs and trusted medical people who clearly were not qualified. Enough said.

FACELIFT SURGERY for SAVING FACE

Often patients ask me: “Do I need a facelift?”  Couples are having surgery together...

Besides not liking the visible age related changes when looking into the mirror, some valuable guidelines can help determine when it’s time for a surgical facelift. Unwelcome Visible signs of facial aging include: lines, wrinkles, deep etched grooves, hollow cheeks and sunken eyes, thin sagging skin, a poorly defined neck/jawline often with “Turkey Wobble”, sagging eyelids and undereye bags and more.  These are the conditions that send men and women to plastic/cosmetic surgeons for a facelift, if injectables and other mini treatments are not appropriate.

AGING and YOUR FACIAL SHAPE:  Each shape affects how a face will age.

  • OVAL FACE … will experience hollowing at the temples and in the cheeks due to fat loss.  Fillers can correct this if started early enough and if needed a Mid-Facelift corrects sagging.

WHAT YOU NEED TO KNOW…BEFORE YOUR SURGERY  –  1) Realistic expectations are essential!  Don’t expect to look 20 if you are 50.  The goal of surgery, in particular, is to  make you look refreshed, more youthful and to fix laxity, hollows etc. Know that a facelift does not always eliminate every single line and wrinkle.   2) Scars are inevitable in spite of our refined techniques and precision instruments.  But, I hide them so often they are not visible while they fade away or blend in.   3)  There is some “downtime.”  For traditional facelift surgery plan on 2-3 weeks before resuming normal activities.  Each case is different, but that’s a reasonable norm. It can take a few months for all swelling to resolve and for your facelift to really settle in.   4)  Learn what can and cannot be fixed/corrected.  The consultation is critical for arriving at an honest understanding of my patient’s concerns, goals and expectations. I believe in educating them as to their options and outcomes.

Sagging cheeks require surgical lifting and tightening of the underlying muscle and tissue and is often coupled with  injectables for ultimate and long lasting results.

INJECTABLE FILLERS and SURGERY   Injectable fillers usually offer immediate and ongoing results as collage production is stimulated and hyaluronic acid fillers replace volume by attracting water.  Surgery results can be subtle or dramatic and last indefinitely.

Choosing Your Plastic Surgeon by Steven Davison M.D. ….DAVinci Plastic Surgery

shutterstock_59992843A recent study conducted for the American Academy of Facial Plastic and Reconstructive Surgeons reported that 80% of those people surveyed said they would prefer and choose a facial plastic surgeon rather than a general surgeon or physician.  As the numbers of people who have cosmetic procedures done each year continues to grow…so does the need for education about how to recognize and choose “qualified” cosmetic/plastic surgeons.  Thus, the reason for my blog topic this time.

Stats tell us there are over 782,000 doctors in the United States but only 3.5% are really qualified to perform aesthetic (cosmetic) procedures.  So, It’s critical that before you go…you know about your doctor/surgeon.

In addition to the right education, training and experience,  Board Certification is important in helping you identify a surgeon who is truly qualified to perform your chosen procedure.  Why?  There are multitudes of doctors who are not Board Certified nor are they members of important medical organizations/associations. So here’s the “why” about being Board Certified:  Certifiying medical boards have rigorous testing/exams based upon the highest standards possible to ensure that a doctor knows, understands and is qualified to practice medicine and perform procedures within the scope of his proven abilities.  There are different boards for different areas of specialization.  The same goes for medical societies and associations that were established decades ago.  These exist to further educate, support and monitor physician/surgeon members, also to educate the population and give them a place to seek help with problems involving member physicians.  Every doctor/surgeon who chooses to become Board certified, must submit to this high standard and the rigorous oral and written exams in order to be eligible for board certification.  Board certification does not guarantee certain results but it does ensure that the surgeon is educated, well trained, skilled and qualified to perform that surgery.

Examples of Certifying Medical Boards and associations:  1) ABMS – The American Board of Medical Specialties (every doctor should apply to this board.)  2) ABPS – The American Board of Plastic Surgery (for certification in the speciality of “plastic” surgery). 3) ASAPS – The American Society for Aesthetic Plastic Surgery (for plastic & cosmetic surgery of the face and body). 4) AAFPRS – American Academy of Facial Plastic and Reconstructive Surgery, Inc.  (World’s largest association of “facial plastic” surgeons and reconstructive surgeons)  These surgeons focus solely on plastic and reconstructive surgery of the face, head and neck. 5) American College of Surgeons, 6) The American Society of Plastic Surgeons is thousands of members strong…with only one high standard to assure you are getting a surgeon who is qualified to perform your procedure.

Do Your Due Diligence  – Surgery of any kind is a serious event and you should want the most qualified surgeon performing your procedure.  You can “google” the doctor/surgeon you are considering and check credentials, qualifications, experience, testimonials and more before making a final decision to use that physician. Surgeon’s Websites should be comprehensive including education, training, medical degree(s), fellowships, experience and qualifications.  Know before you go!  https://www.davinciplastic.com.

Cosmetic v Plastic Surgery…there is a difference.  Cosmetic surgeons have considerably less training whereas a “plastic” surgeon gets 5-7 years of specialized surgical training.  Cosmetic surgery seeks to improve the aesthetic appearance and Plastic surgery is used to improve/restore function and reconstruct after trauma or illness.  Plastic surgery can include “cosmetic” procedures also.

Ask the right questions:  1) Is the surgeon Board Certified…which boards and for how long?   2) Does he/she have hospital privileges?  3)  Is their surgical suite certified?  4)  Does the surgeon practice within the scope of their chosen field?

Revision surgeries are on the rise…because unqualified doctors and beauty beauty professionals are doing procedures they have no business doing! Every year, my colleagues and I see more patients desiring “revision” surgeries to undo the disappointing, disfiguring damages inflicted upon them by the hands of unqualified doctors and so called professionals.  Please trust your face and body only to a qualified plastic surgeon!

Revision Rhinoplasty (Nose Job) corrects undesirable form and function from a previous surgery

Since I’m specially trained and very experienced in reconstructive and revision surgeries I do a lot of “Nose Jobs.”  Patients of all ages, stages in life and from all cultures come to me because they are unhappy with a previous nose (rhino) surgery.

Rhinoplasty surgery is done to change the size, shape  and form of the nose and/or to correct functional problems such as difficult breathing caused by a broken nose (deviated septum), fracture or disease.  So, when the outcome from a nose surgery is not desirable, often I am asked to fix it.  Following are some very important points you need to know and understand about revision nose jobs.

Revision Rhinoplasty is always more challenging than the original (primary) rhinoplasty surgery.  Several reasons for this are as follows:

  1. The geography of the nose was altered during the first surgery.  This involved removal of cartilage which provides form and structure for the nose
  2. The cartilage needed for the revision surgery must then be borrowed from someplace else
  3. An extra incision is required to allow for borrowing the needed cartilage
  4. The nasal lining has typically turned to scar tissue in the operated nose
  5. Risk of little spider veins increases with a second surgery (telangectasias)
  6. Revision Rhinoplasty is very complex and detailed since nasal form has to be restored and function (breathing mechanisms) have to be repaired
  7. These challenges increase the time involved in the surgery
  8. Very Few surgeons are qualified to perform these highly specialized revisional surgeries

Even though Revision Rhinoplasty has its own set of unique considerations, it is among the most rewarding of all surgeries.  The nose is the most prominent feature on the face so getting it right is most rewarding to people having this surgery.

HOW I PERFORM REVISION RHINOPLASTY:  There are 2 methods used for nose surgeries:  Open Technique and Closed Technique. Open involves incisions made inside the nose and 1 under the nose between the nostrils.  Closed means incisions are inside the nose only.

Revision Rhinoplasty surgery is frequently performed using the open approach which allows for more “complex” nose jobs to be corrected with natural looking, desirable outcomes.  Borrowed cartilage is taken from behind the ear, when needed,to restore nasal form and function.  This incision is well hidden.  The under the nose incision is barely visible and fades to a very faint thin line over time.

Here are 3 frequent questions I’m asked:

1. About anesthesia…I use local with sedation or general.  We determine that during your consultation to choose what’s best for you.

2.  Who can have Revision Rhinoplasty?  Men and women of all ages (must be in good health) and teen boys and girls who are fully grown.

3.  How long does the surgery take?  This depends upon the complexity involved.  It could be from 45 minutes to 3 or 4 hours.  The important thing here is to achieve a pleasing outcome with restored function.
