Top “Tweakment” 2019: Non Surgical Rhinoplasty

“Non surgical nose jobs” have been popping up more and more on Instagram over the last year. As awareness and interest increases, the amount of people asking for this procedure, formally called Non Surgical Rhinoplasty, is bound to increase simultaneously. It is positioned to be one of the top “tweakments” of 2019.

What is a “tweakment”?

A tweakment is an adjustment to your looks to align it with your ideal self. It’s not a complete change of who you are or what you look like, but a small change to enhance your existing features. Tweakments are usually less invasive procedures (though sometimes still surgical) and don’t involve as much recovery or downtime. These usually are performed to help your actual appearance line up with the filtered, photoshopped self on social media.

What is Non Surgical Rhinoplasty?

Rhinoplasty is one of the most popular surgeries because the nose is a flagship feature of the face. When the nose is put into facial harmony, it offers one of the most greatly satisfying results. The change can be quite drastic to one’s appearance, but it usually what is desired. On the other hand, the Non Surgical Rhinoplasty is a quicker, no downtime route to achieving your preferred nose. It uses injectables to achieve the desired small changes, so there is no sedation or recovery time. With topical numbing and extremely precise injections over the course of a lunch break, you’re on your way.

What can Non Surgical Rhinoplasty do?

The injectables fill in curves and bumps, adjust the shape of the tip, and increase the hight of the bridge, which minimizes the appearance of width. Even though hyaluronic acid filler is adding volume, the precisely placed injections can actually make your nose look smaller and straighter.

Who is not a candidate?

Someone who has already had Rhinoplasty or a nose break, may not be a candidate depending on the fragility of the nose blood vessels. Also, if the desired adjustment to the bump or width of the nose is just too drastic, traditional Rhinoplasty would be needed to achieve desired results.

Any downsides?

As with any injectable treatment, there are risks and the results are temporary. Results usually last around a year, give or take, depending on the filler used. Also, if price is a factor then, Rhinoplasty is more cost effective in the long run due to the temporary nature of the fillers. However, some people are willing to incur the cost in order to avoid surgery.

Using injectables for Non Surgical Rhinoplasty is called an “off label” use of the hyaluronic fillers, so this use of them hasn’t been endorsed by the FDA and comes with health risks. For safety reasons, treatments should only address concerns along the bridge of the nose and only be done by expert injectors familiar with nasal anatomy. This ensures that any blood flow is not inadvertently blocked, which can have severe negative affects to the eyes and surrounding skin.

Getting it right: REVISION RHINOPLASTY

 

Very few surgeons are truly qualified to perform the complicated procedure of Revision Rhinoplasty. Revision or Secondary Rhinoplasty is a correction, whether functional or aesthetic, to the unsatisfactory results of a previous Rhinoplasty (Nose) surgery.

WHEN TO GO FOR THE REVISION… Typically, the recommendation is to wait a year after the original Rhinoplasty so that there is ample time for healing and the nose to “settle” into its new anatomy. There can be a shorter duration to this recovery time depending on the extent of the previous surgery, which can be discussed during the consultation.

Although the decision should not be made lightly, you should consider a revision if you have any of these issues:

  • persistent breathing problems
  • nose still too large
  • still a bump in the bridge
  • new nose not in balance with the rest of the face
  • too much removed
  • tip of your nose needs better refinement
  • tip is deflected off to one side (“twisted tip”)

WHY IS REVISION RHINOPLASTY MORE COMPLICATED?

  • Changes to the typical anatomy and geography of the nose due to cartilage removal during the first surgery
  • There may not be enough cartilage in the nose to work with and it has to be borrowed from behind the ear, which also requires extra incisions
  • Navigating the scar tissue in the nasal lining from the primary surgery
  • There is a higher risk of developing little spider veins in the nose called telangectasias
  • All of these issues increase the variables and the amount of time in surgery

SURGICAL EXPERIENCE IS CRUCIAL… It is possible that the problem resulting from the original Rhinoplasty may not be the fault of the original surgeon. Anatomical variations and the healing process aren’t under their control. Although, the actual surgery is under the surgeon’s control, which is why you need the most experience surgical hands for the best possible outcome. This is especially true with navigating the altered anatomy and scar tissue present during a revision Rhinoplasty. It requires not only skill, but an artful eye, as well.

Breathe Easier with Rhinoplasty

Do you have trouble breathing through your nose? One of the possible causes is a deviated septum. This can happen as a result of genetics or an impact caused by an accident or athletic event. A deviated septum makes breathing difficult because the bone and cartilage that is supposed to be down the center of the nose that divides the nasal cavity in half is crooked or drastically off center.

  • The best way to confirm a deviated septum is to see a specialist like myself. The following are common symptoms of a deviated septum:
  • nasal congestion on both sides or one side being more congested than the other
  • difficulty or loud breathing
  • repeated sinus infections or constant postnasal drip
  • aching in the head or face
  • issues with breathing during sleep, such as snoring or sleep apnea
  • nosebleeds

The specific surgical procedure to fix a deviated septum is called a Septorhinoplasty.
TREATING A DEVIATED SEPTUM

Septoplasty is often combined with Rhinoplasty, or nose job, to correct the external effects of the crooked insides. The combination of these two procedures is called a Septorhinoplasty. Rhinoplasty is one of the most common surgical procedures and in the hands of a skilled surgeon like myself, the results can help you breath easier from both physiological and aesthetic improvement. Anyone in good overall health is a candidate, although for children needing septoplasty, it is preferable to wait until after the nose has stopped growing around age 15.

WHAT DOES SEPTORHINOPLASTY INVOLVE?
Septoplasty: By working through the inside of the nose, a small incision is made in the septum. The breathing canals of the nostrils are evened out by removing the excess bone or cartilage. Soft gauze or internal splints stabilize the nose as it heals.
Rhinoplasty: This portion of the procedure corrects any cosmetic imbalances in the bridge or nostrils to improve the nose’s appearance and overall harmony of the face. This recontour of the nose improves functionality and appearance to give optimal performance and confidence.
Both are performed on an outpatient basis and using either intravenous sedation or general anesthesia, depending on the patient’s health conditions and preferences.
RECOVERY… Oral pain medication will help mitigate the discomfort from either surgery. Septoplasty recovery should be within a week. Rhinoplasty recovery from bruising, swelling, and incision healing is longer than Septoplasty alone. It’ll last about a week or two, although some people can return to work after 1 week. Either way it’s recommended to rest of the head elevated to help with swelling. Even after the initial Rhinoplasty recovery of about 2 weeks, patients should refrain from intense exercise and contact sports for at least 1 to 2 months. This makes it imperative for athletes to correct their deviated septum in the “off season.”

Have you mastered the art of taking a “selfie” yet? These self portrait photos, taken on cell phones, are over populating the newsfeeds of Facebook and Instagram. With the proliferation of the selfie comes a higher self consciousness of the actual image we portray of ourselves. More now than ever, people see pictures of each other through social media on almost a daily basis.

COSMETIC PROCEDURES DUE TO RISE IN SELFIES… One in three plastic surgeons found an increase in procedure requests due to patients’ social media awareness per a recent poll by the American Academy of Facial Plastic and Reconstructive Surgery. Blepharoplasty, rhinoplasty and hair transplants had increased from 6 to 10% from 2012 to 2013.

SELFIE SPECIALTY SURGERIES… Straight noses, bigger breasts and flatter stomachs are still in high demand. And, plastic surgeons are now being asked to correct imperfections that have been brought to our attention due to “social media selfies.” Posting a nice photo of you wearing your new sky high heels will show off your calves, feet, and ankles. If you’re unhappy with how these look, your concerns can be addressed with a calf augmentation; while trimming the ankle fat with microliposuction or even injections in the toes for nice toe cleavage. LONG LASTING FACIAL MAKEOVERS

Rhinoplasty — Surgery of the Nose

I do a lot of nose surgeries.  It is one of the most common and important surgeries we do.  Every year over 1.5 million men, women and children of all ages and ethnic groups have nose surgery for cosmetic and functional reasons.

So, let me talk a little about it today.

The first nasal reconstruction was performed in India where removal of the nose served as punishment for various crimes.  That’s not how and why I do nose surgeries!

Rhinoplasty is one of the most complex, delicate surgeries performed today. The primary goal is a natural looking nose that complements all of the other facial features by balancing its size, width and angles. 

These are some of the reasons people come to me for nose surgery:

  • Correct or improve appearance and function
  • Reconstruction to correct damage from injury or a precious surgery

“Can you achieve the perfect nose?”  Rhinoplasty is a combination of art and science.  I do a thorough examination of the facial anatomy, appearance, proportion and structure.  Then I match the nose to the rest of the face…in other words…I sculpt your nose to achieve natural facial balance.

“How is it done?”  This is where my years of experience, skill, artistic ability and today’s advancements enable me to get the job done right.   No two faces are alike…your nose must fit your face naturally. I perform either an open or closed procedure (see my website) and I work through the incisions to remove excess tissue, reshape cartilage and bone and alter the shape and size of the nose.

Pippa’s (nose) Tip

Pippa's Tip

It is common for patients to ask for a rhinoplasty or nose job to look like a certain celebrity. Even more common is I won’t want a nose like Michael Jackson. The most asked about nose job likeness is now Pippa Middleton. Let’s analyze what makes her nose attractive:

  1. From the front view, the nose has very symmetric sides that arise from the eyebrow and gently sweep down to the ala where the cheek meets the nose.
  2. The dorsum is straight, not scooped – it is strong and doesn’t look done.
  3. There is good “supratop break”. This is a dip behind the tip before the dorsum starts. This isolates and defines the tip.
  4. The top is well-defined with a double point where the nasal cartilage throws off points of light.
  5. The base of the nose is not too wide and not too pinched. As Goldilocks would say – it’s just right.

How do I get Pippa’s tip, you might ask? Modern rhinoplasty techniques, particularly “open” rhinoplasty where sutures and grafts are used to define the nose can help give you the refinement her nose exudes.
