LIPOMATOSIS…is the presence of multiple lipomas on the body.  Lipomas are adipose tumors….these are tumor growths filled with mature fat cells.  Lipomatosis is also referred to as: liposis or adiposis.  Whatever it is called…it simple means an abnormal localized tumor like accumulation of fat in the tissues.  Many discrete, encapsulated lipomas form on the body trunk and extremities with very few on the head.  In 1993, a genetic defect within lipomas was localized to chromosome 12q15, where the HMGIC gene encodes the high-mobility-group protein isoform 1-C.  This is one of the most commonly found mutations in solitary lipomatous tumors, while lipomas often have multiple mutations.  Other chromosomes have also been observed.

Other entities often accompanied by multiple lipomas include Proteus syndrome, Cowden syndrome and related disorders due to PTEN gene mutations, benign symmetric lipomatosis (Madelung disease), familial lipodystrophy, hibernomas and familial angiolipomatosis.  Below I outline some symptoms….because more people are presenting today with lipomas….it is important to educate on this subject.

SYMPTOMS of FAMILIAL LIPOMATOSIS (FML)…FML is a rare disorder that tends to run in families and affects both men and women.  It is hereditary and shows up as groups of fat cells in the subcutaneous tissues that begin to grow and produce multiple fatty lumps of “benign” fatty tumors (lipomas).

• Usually appear on the abdomen, arms, thighs, neck
• Can also appear on the face, buttocks, lower legs
• Tend to vary widely in size…& may or may not be surrounded by connective tissue
• May or may not be attached to the skin & nearby muscle
• They are rubbery, moveable & ovoid to round in shape
• Are not usually painful (unless in an irritated area)
• Can appear singularly or in clusters
• Typically appear during of soon after adolescence
• Extreme cases can grow to appear grotesque & can interfere with mobility, movement & cause disfigurement

Lip Augmentation…the right way to enhance the lips with hyaluronic acid fillers or fat (fat is great) adds volume to make them bigger.  The key is in not adding too much volume because it can make them look “overdone” or like “duck lips” and facial balance is lost.   I’ve written about facial balance…see recent blogs to get up to speed on that.

Female Genital Cosmetic surgery is not new…it just hasn’t been talked about much.  Women used to be shy and embarrassed to discuss intimate health concerns with their physician let alone sexual issues. STAGE TWO BREAST RECONSTRUCTION

From previous blogs you know that Stage I involves placement of the Tissue Expander…often placed under the same anesthesia as your mastectomy surgery… and called Immediate Breast Reconstruction.  Delayed Reconstruction means you choose to wait a few weeks or months before starting the restoration process.

Here are the steps leading up to Stage II Reconstruction:

During the mastectomy skin and breast tissue are removed, leaving the chest tissue flat and tight.  The “expander” is inserted under the chest muscle and skin to serve as a “space saver.” The expander is slowly filled with saline solution (over many weeks) to stretch the remaining skin and muscle to allow for the future breast implant.

Expander surgery is done under the same anesthesia as the mastectomy surgery.  (Immediate Reconstruction)  Healing takes 4-6 months

NOW Stage II begins: The new Breast Implant is placed when Dr. Davison determines the space is sufficient and then the expander is removed.  This is done through the original incision.

Stages III & IV …Begin about 3 months later and Involves A new nipple and areola…with additional breast beautification.

I become your primary care physician during this entire process…generally about a year.

Stage II Breast Reconstruction involves rebuilding a new breast mound by using what is called a Flap Procedure.  There are several options for flap procedures that I will cover in my next blog.

A quite frequent problem involved in the removal of small lesions such as moles, nevi and other lesions is…correct identification.  Correct identification and confirmation of the exact site for these small lesions is paramount.  But often, this is a problem when a patient is referred from a dermatologist and presents in our office.

Identification and Clinical Notes The most common way of locating and identifying the correct site is by asking the patient to identify the site in conjunction with clinic notes.  This method is used to help maximize accuracy in the ID process.  Problem is…this can backfire.

I’ve actually had a patient blow up at me when I asked them to confirm which mole the dermatologist wanted biopsied.  But there is an easy solution to this problem.

Smart Phone’s High Pixel Camera! The Smart Phone’s high pixel camera does not require additional lighting, so you can take a photo in the dermatologist’s office without hassle.  Here are the steps I recommend:

1. Take a photo (or have someone on staff do it)…of the marked lesion.
2. Include a frame of reference – e.g. the nose, right ear, left ear, belly button – something discernible.
3. If the lesion is hard to find, include a ruler located by the lesion so we can measure from the landmark.
4. This prevents hours of anguish caused from having to revisit the dermatologist and from incorrect lesion biopsy mistakes.

Men today are getting more and more cosmetic procedures done…for various reasons.  Facelift is one of them.  Since facelift surgery is so popular today…with a growing male clientele, I want to encourage the guys with some info.

My last blog covered the six basic techniques used for surgical facelifts.  So, please refer back to that if you have related questions.  Also, see my website for in depth information on each procedure.

Here’s why Men have facelifts today:

• Want to match their physical appearance with how old they feel.
• Are looking older than they are…and decide to correct it.   This happens when a dad is asked if his son is his grandson.  ZING!
• Men want to look “as good” as their wives who are getting things done.
• They’re going on job interviews  (may have lost a job) and they want to do all they can to “compete.”
• Aging, injury, trauma, disease have taken a toll “emotionally” and they need to boost their self esteem and self confidence.
• Pride.  Some men just never want to look their real age.  Always want to look great!
• Stigma…it’s gone.  It’s OK and men know it.

How Old are these Men? They are of all ages and stages in life.  The critical thing about “age” and facelift surgery, is that the older client should be in overall good health and must expect different results which may take a little longer.  The risk factor is no greater in people over 65 as long as they are healthy with realistic expectations. Men today know that starting in their 40’s/50’s can keep them looking good – always.

If married – what do their wives think? Their wives love it!    A good facelift job needs to match the individual and look “natural” while taking several years off the face.

DOES LOOKING BETTER REALLY HELP YOU GET THAT JOB? This is always a sensitive subject, especially in these economic times, but if you believe “the truth can set you free” (and I do) then this information should be helpful to many people.

If youth equals beauty – then beauty in turn equals money. People in the work force, and those trying to re-enter the career arena, are stampeding cosmetic clinics, like mine, for a host of procedures that will lift years off the face and neck.

Why is this happening during an economic downturn? Because they have to look good (younger) in order to compete in the job market place.  Here are the numbers:

• 2009-2010…cosmetic procedures rose more than 5%
• Facelifts for men were up 14% in 2010
• Non-Invasive procedures rose more than 12%
• England & Ireland (in spite of their economic woes) saw an increase among women having Botox® – by more than 50%
• Career counselors are telling clients to:  spend at least as much $$ on looking good as they spent on perfecting their resume
