Summer Sun Fun – Part 2

I shortened my last blog so I could wrap up with the UVA/UVB info that is so often mis-understood.  It is important to understand the following things:

The Long (UVA) and Short (UVB) of It
While UVA rays are longer and less intense than UVB, we are exposed to more throughout our lifetimes because they are equally present during all daylight hours throughout the year.

UVA can also penetrate clouds and, unlike UVB, can go through glass. This is why it’s important to consider sun protection even when you’re not at the beach or pool.

It has been thought that while both play a major part in skin aging, UVB has been the main contributor to skin cancer. UVB is the chief cause of skin reddening and sunburn, affecting mostly the superficial layers of skin.

But even though UVB has a more concentrated negative effect, UVA causes cumulative damage over years and years, which may also initiate the development of skin cancer. Most skin cancers occur in the basal layer of the epidermis and studies have shown UVA damages cells in this layer, as well.

Bottom line is that while the sun provides wonderful health benefits, we still need to take precautions to only receive its benefits and not its damaging effects. In my next blog, I’ll continue the subject and talk about how to protect yourself, including helpful clothing, sunscreen and diet tips.

Safe Sun Protection Part 2

The sun provides some wonderful health benefits, but as I’ve recently shared in previous blogs, there is good reason to protect yourself from its skin damaging UV rays. Here are the main ways to protect yourself when in the sun.

Sunscreen
Sunscreen can definitely be helpful in protecting your skin from the UV rays, but some sunscreen has toxic chemicals that should be avoided. For example, a recent study published in Environmental Science Technology has demonstrated that the common sunscreen ingredients, methoxycinnamate, oxybenzone and PABA are estrogenic chemicals linked to cancer.

Mineral sunscreens are preferable, as they are considered “broad spectrum” and protect against both UVB and UVA rays. They contain titanium or zinc, which do not breakdown in sunlight and are therefore more effective at also blocking UVA rays. These minerals are non allergenic and not usually absorbed into the bloodstream so they don’t affect the body’s hormones.

Clothing
You can’t go wrong by wearing a hat, sunglasses and protective clothing when in the sun. There is even special clothing that is enhanced with greater protection against UV radiation and laundry detergent that adds sunscreen to your clothes. The laundry detergent doesn’t add much, but a plain white Tshirt that normally only provides 8 SPF can be boosted up to 15 SPF with a sunscreen detergent. The main benefit to SPF laundry detergent is that it’ll help keep some SPF in the garment for longer, as it does tend to start to slowly wear off after 3 or 4 washings.

UPF vs SPF
Most commonly you’ll see the terms UPF (Ultra Violet Protection) on sun protective clothing, while the sunscreens say SPF (Sun Protection Factor). UPF measures a fabric’s protection against both UVA and UVB. Sunscreen’s SPF only rates its protection against UVB rays, which cause sunburn.
What does the number mean?

A UPF rating of 25 means that only 1/25th (or 4%) of the UV radiation can penetrate the fabric, giving a 96% protection against both UVA and UVB rays. A minimum UPF 15 clothing is good (94% protection), but 50+ (98%) is excellent, especially considering the effectiveness of sun protection will lessen over time, you’ll have longer lasting protection.

Regarding the measurement of sun protection, SPF is a bit more variable as the number is calculated based on increasing the amount of time you can spend in the sun before getting sunburned. For example, if the average person would burn after 20 minutes in the sun, a sunscreen with SPF 30 is supposed to protect against sunburn for about 10 hours. This effectiveness varies widely because some people’s skin burns more easily, the mid day hours have more intense UVB rays, and even most water repellant sunscreens wear off with sweat and water. Also, the SPF calculation does not account for the UVA rays.

Diet

Most people haven’t considered that foods and supplements that can actually help protect your skin, too. The American Cancer Society states that oxidative damage can increase your cancer risk, including skin cancer. Fortunately, there are lots of delicious, healthy foods that help reduce oxidation.

These are some suggestions for foods and the antioxidants they contain that help your skin:
Salmon and fish oil contain astaxanthin and omega 3 fatty acids.

  • Green and white tea contains catechins.
  • Red wine, blueberries and red grapes contain resveratrol.
  • Almonds, asparagus and pumpkin seeds contains Vitamin E
  • Carrots and red bell peppers contain Beta carotene.

Skin Cancer Detection & Prevention

Summer is on its way and people will get more exposure to the suns’ rays than any other time of year. We’ve all heard the word of caution about too much sun causing skin cancer, but specifically it’s the Ultraviolet (UV) light exposure that is overwhelmingly the most frequent cause of skin cancer. Since May is Skin Cancer Detection & Prevention Month, and skin cancer affects more than 1 million people in the United States each year, I thought I’d dedicate some time to the topic.

Skin Cancer Causes
Other than frequent, unprotected sun exposure, some significant causes of skin cancer include:
1.  The frequent use of tanning booths, which, like the sun, expose the skin to harmful ultraviolet (UV) rays.
2.  Immune system compromise, which impairs the body’s ability to fight disease. A suppressed immune system affects the ability of the body to regenerate and repair at the cellular level. Ways to improve the immune system are to stress less and laugh more, drink more water and less alcohol, eat antioxidant rich foods rather than empty calories and sugar, as well as take a probiotic to promote beneficial gut flora.
3.  Regular exposure to electromagnetic wave of high energy and very short wavelength, such as Xrays. And exposure to certain chemicals, such as arsenic and those in tar, oils and soot.

People with Greater Risk
– Besides outside environmental factors, certain people are predisposed to skin cancer. These are the characteristics of the people who are at greater risk:
Fair skin that burns easily from the sun exposure and freckles easily
Blond or redheads with blue or green eyes
Depleted skin pigmentation, such as from certain genetic disorders like albinism and xeroderma pigmentosum.
Previous cancer or close family members who have had skin cancer
Birthmark of a large mole, or numerous moles and unusual moles that developed later in life
– Experienced a severe sunburn in childhood

“ABCD’s” of Cancer Detection
1.  Asymmetry: one side not looking like the other
2.  Border irregularity: rough, jagged or bumpy edges
3.  Color: mixture of black, tan, brown, blue, red, or white
4.  Diameter: larger than the size of a pencil eraser (about 6 mm across) or noticeable increase in size

Kinds of Skin Cancer
The skin is the largest organ in the body and skin cancer is the most common of all cancers. There are three major kinds of skin cancer:
Melanoma,
Basal Cell Carcinoma (BCC)
Squamous Cell Carcinoma (SCC)
Melanoma is actually the most frequent cancer in people age 25 to 29 years. About a third of all melanoma are diagnosed before the age of 50, although most skin cancers are diagnosed between 50 and 75 years old. Squamous cell carcinomas and basal cell carcinomas, which are the vast majority of skin cancers, are more common in older people. Unlike the Melanomas, the BCCs or SCC usually do not spread to other parts of the body.

Prevention
It is important to self check using the “ABCD’s” and to regularly be checked by a dermatologist.

A doctor can send away a biopsy of a questionable mole or marking to tell if it’s cancerous.

Fortunately, you can reduce your risk of getting skin cancer. This is one of the main reasons why I recommend safeguarding yourself when you enjoy the beautiful weather with sunscreen, protective clothing, sunglasses and a hat.

WOMEN, POLITICS/BEAUTY, BREAST HEALTH, TATTOOS and LASERS

shutterstock_10125679Women in politics are constantly in the spotlight and can’t hide their appearance and cannot avoid being frequently analyzed from every angle.  Hillary Clinton is just one example of many women in politics, or the entertainment field who are under fire over their appearance and the stigma behind the idea of these women seeking anti aging cosmetic surgery/treatments for a “refreshed” looking face.  Betty Ford is one of the bold ladies  who had cosmetic surgery decades ago and discussed it for the benefit of all women.  She chose a fresh new face to go with her beautiful life!  Hillary Clinton did likewise and others have followed. Hollywood celebs such as Nicole Kidman and Jane Fonda also blazed the trail for women and they helped remove the “taboo” against age defying cosmetic procedures.

One of the arguments has been that surgery isn’t just about improving your looks,  it is also about improving your self esteem and self confidence. Removing grumpy, fatigued, stressed, sad/angry facial aging can be positively life changing.

Ultimate AntiAging Road Map

A good looking nose improves the facial profile allowing certain features to pop out more.  Surgery is needed when there are functional issues and flaws that fillers can’t handle. NOSE FLAWS MEN WANT CORRECTED include a deviated septum, bulbous nose tips, crooked noses from being broken, pitted and scarred areas, humps and bumps from injuries or birth and others. Functional correction is key with men.  They don’t like noses that don’t work right.  So, I do many Rhinoplasty surgeries and non surgical nose jobs also.

Remember the “fun in the sun” tips….lots of water to drink (stay hydrated), sun screen, umbrellas, anti oxidant snacks like raw fruits, sun glasses, avoid glare from the water…the sun is hottest between 11:00 am and 2:00 pm.

Revision Rhinoplasty (Nose Job) corrects undesirable form and function from a previous surgery

Since I’m specially trained and very experienced in reconstructive and revision surgeries I do a lot of “Nose Jobs.”  Patients of all ages, stages in life and from all cultures come to me because they are unhappy with a previous nose (rhino) surgery.
Couples are having surgery together...

Rhinoplasty surgery is done to change the size, shape  and form of the nose and/or to correct functional problems such as difficult breathing caused by a broken nose (deviated septum), fracture or disease.  So, when the outcome from a nose surgery is not desirable, often I am asked to fix it.  Following are some very important points you need to know and understand about revision nose jobs.

Revision Rhinoplasty is always more challenging than the original (primary) rhinoplasty surgery.  Several reasons for this are as follows:

  1. The geography of the nose was altered during the first surgery.  This involved removal of cartilage which provides form and structure for the nose
  2. The cartilage needed for the revision surgery must then be borrowed from someplace else
  3. An extra incision is required to allow for borrowing the needed cartilage
  4. The nasal lining has typically turned to scar tissue in the operated nose
  5. Risk of little spider veins increases with a second surgery (telangectasias)
  6. Revision Rhinoplasty is very complex and detailed since nasal form has to be restored and function (breathing mechanisms) have to be repaired
  7. These challenges increase the time involved in the surgery
  8. Very Few surgeons are qualified to perform these highly specialized revisional surgeries

Even though Revision Rhinoplasty has its own set of unique considerations, it is among the most rewarding of all surgeries.  The nose is the most prominent feature on the face so getting it right is most rewarding to people having this surgery.

HOW I PERFORM REVISION RHINOPLASTY:  There are 2 methods used for nose surgeries:  Open Technique and Closed Technique. Open involves incisions made inside the nose and 1 under the nose between the nostrils.  Closed means incisions are inside the nose only.

Revision Rhinoplasty surgery is frequently performed using the open approach which allows for more “complex” nose jobs to be corrected with natural looking, desirable outcomes.  Borrowed cartilage is taken from behind the ear, when needed,to restore nasal form and function.  This incision is well hidden.  The under the nose incision is barely visible and fades to a very faint thin line over time.

Here are 3 frequent questions I’m asked:

1. About anesthesia…I use local with sedation or general.  We determine that during your consultation to choose what’s best for you.

2.  Who can have Revision Rhinoplasty?  Men and women of all ages (must be in good health) and teen boys and girls who are fully grown.

3.  How long does the surgery take?  This depends upon the complexity involved.  It could be from 45 minutes to 3 or 4 hours.  The important thing here is to achieve a pleasing outcome with restored function.

1.  A stumpy shadow means the sun is directly overhead and UVB exposure is up to 50% greater during this time.  2. Wear hats that throw a shadow over your face and neck.   3. Be generous with sunscreen…a thin layer does nothing!    4.  Summer white/pastel gauze type fabrics absorb rays because they lack color pigments…when wearing them wear a layer of sunscreen also.  5. Apply sunscreens indoors….to allow them to “bind to” skin proteins….otherwise they don’t work!   Come back next blog for more tips!

To Wake up Every Day to beautiful skin…know these myths and tips.

Some skin care myths You need to know about…We carry only the best skin care products in our spa…and our lifelong clientelle have learned certain truths about healthy skin, and they use products that really work.

Myth #1. After washing it, your skin should feel tight. That means it’s clean. Answer: tight skin after cleansing has been stripped of all hydration causing dead skin cells to build up on the surface.  Avoid stripping cleansers (e.g. bar soap).

Myth #2.  Greasy foods and chocolate are offenders and cause breakouts.  Answer: This may inspire you to eat healthier, green foods, but there is no science to support this myth.

Myth #3.  Smoking is bad for your lungs…but has no negative effects on your skin.  Answer:  WRONG!  Smoking narrows the tiny blood vessels in the outer layer of the skin and it depletes the skin of essential oxygen and nutrients that promote health.  The damage shows up clearly as: facial lines, wrinkles, folds (e.g. smoker’s lips) and discoloration…not to mention lung diseases.

Myth #4.  Longterm, frequent use diminishes effectiveness of skin care products.  Answer:  The skin is in a constant state of renewal as old, dead skin cells are pushed to the surface and new cells are generated about every 30 to 40 days.  Good skin care products enhance this process to create healthy, beautiful skin.

Myth #5.  You don’t need sunscreen it your makeup has it.  Not all makeup is created equal…so don’t take a chance on your precious skin.  Use sunscreen for play in the sun times.

How Do They Work?  

Injectable fillers like Radiesse work immediately by replacing volume and stimulating your own collagen renewal while attracting water to the treated area. Juvederm is used in areas requiring less structural improvement…such as under the eyes and for marionette lines. Botox is used to inhibit muscle action allowing forehead furrows to relax and smooth out and also prevents future lines and wrinkles.

Here are a few more tips on Melanoma…since May is National Skin Cancer awareness month.  See my last blog for detailed information on the three types of skin cancer and most critically… melanoma skin cancer.

SCREENING:  Melanoma can be deadly but is treatable with immediate diagnosis and early treatment.  After Melanoma spreads to the lymph nodes, the 5 year survival rate possible with treatment, drops to 62% and 15% respectively.

You can self-screen to check for skin cancers or you can see a dermatologist or sign up for a “free” cancer screening.  They are offered frequently around the country.

SELF CHECK for MELANOMA:

Use these steps:

1) STAND UP & look into the mirror & examine your head, scalp, face, neck, chest, torso, (women under the breasts), hands, nails, elbows, arms and underarms.

2) SIT DOWN & check your legs, feet, soles, heels & toenails.  Use a HAND MIRROR to check genitals.

3) STAND UP & use a HAND MIRROR to inspect the back of neck, shoulders, upper arms, back, buttocks & legs.

REDUCE YOUR RISK:  We cannot completely avoid sun exposure, but there are ways to reduce the risk from exposure.

COVER UP…Wear cool, clothing that covers your body.  Dark colors block more UV rays than light colors.

WEAR A HAT…Wear a wide brim hat when out in the sun. It should cover your ears, eyes, forehead, nose and scalp.  A baseball cap is better than nothing.

SUNGLASSES… invest in good glasses that block UV rays.  They should block 99 t0 100% of UVA & UVB rays.

SUNSCREEN…LOTS OF IT!  Get it, use it liberally and apply it often…at least every 2 hours when in the sun.

May is National Melanoma/Skin Cancer Detection/Prevention Month…Early Detection Increases Chances for Survival

One in every five Americans will develop skin cancer during their lifetime and every hour…one person in America dies from skin cancer.  Melanoma is the most deadly form of skin cancer and it is preventable/treatable.

May has been designated as National Skin Cancer Awareness month by several organizations such as the National Cancer Institute, Skin Cancer Foundation, Centers for Disease Control and the American Academy of Dermatology.  I won’t belabor the facts…just want to remind you of them and offer some helpful and important tips for skin cancer prevention.

Review of Skin Cancer Facts

-During the month of May we try to raise the public awareness of the importance of prevention, early detection and immediate treatment of skin cancer.

They are:  1) basal cell, 2) squamous cell 3) melanoma.

-More than 3.5 million skin cancers are diagnosed annually in more than 2 million people.

-Melanoma (unlike other cancers) has a wide age distribution…it occurs in young people & seniors.

-Rates of occurrence increase with age- they are highest among 80 years olds.

-One bad burn during childhood doubles the risk for getting melanoma skin cancer later in life.

-Skin cancer is the most deadly for African Americans, Asians & Latinos.

RISK FACTORS

Everyone is at risk…but some people with certain factors are more likely than others to develop skin cancer.  The risks include:

Lighter natural color skin

Family history of skin cancer

Personal history of skin cancer

Exposure to the sun through work & play

History of sunburns early in life

Skin that burns easily freckles, reddens easily or becomes painful in the sun

Blue or Green eyes

IMPORTANT TIPS FOR PROTECTION/PREVENTION

1.  AVOID the sun between 10am and 2pm…this is the when the rays are at their peak

2.  Damage occurs all day long…so be wise and use sunscreen or sunblock

3.  Take more precautions when it is hot, humid and windy as these conditions enhance harmful effects of   UV rays

4.  Cloudy days are dangerous days…we tend to be out more, longer and use less protection.  Understand that UV Rays pierce the clouds and cause serious damage (burns)

5.  TANS are NOT healthy!  No tan, whether from sun exposure or tanning beds, is healthy.  The darkened skin is merely a protective measure (performed by your skin).  It provides only minimal levels of protection and does not cancel out the need for using other sun protective methods.

6.  Short Term Symptoms of over exposure include: blistering, peeling, tanning

7.  Long Term Symptoms= increased risk for skin cancer along with dry, wrinkled skin.

Early diagnosis & immediate treatment of melanoma cancer can mean the difference between life and death.
