Breast Lift Incisions Explained

I wrote some blogs on breast surgery a few weeks ago and then took a break to focus on summer sun precautions.   Now I want to finish the breast series with this blog on scarring and incisions.  Scarring is always a concern when choosing a cosmetic surgery. We want to be able to focus on the beautiful results of the surgery, not the incision scars it may leave behind. Fortunately, with a Breast Lift, there are a variety of incisions, some more inconspicuous than others. However, different techniques may be required based on how much sagging you want corrected. The incision I recommend will be based on your personal goals, existing breast tissue, and the amount of excess skin to be removed. Here is an explanation of each of the possibilities.

Minimal Sagging: Crescent Incision
This incision is very small and traces around the upper half edge of the areola. While this is minimal scarring sounds ideal, it can take away only a minimum amount of skin. This makes it only an option for patients who have very little sagging to correct. For this reason, it is the least frequent incision technique and is usually only done in combination with a breast augmentation.
Mild Sagging: Peri Areolar or “Donut” Incision
The Peri Areolar incision is a circular incision tracing around the outer edge of the areola. This technique can also be used to reduce the size of the areola if that is desired. Like the Crescent incision, this “Donut” incision is commonly performed along with a Breast Augmentation and can address mild sagging.

Moderate Sagging: Vertical or “Lollipop” Incision
Two incisions are required for this technique, as it can accomplish more extensive reshaping of sagging breasts. They form the pattern of a lollipop, hence the nickname. The first incision is around the outer edge of the areola and the second runs down vertically from the bottom center of the areola to the underlying crease where the breast meets the torso, called the inframammary fold. This is a very common incision technique since it allows for the removal of a moderate amount of excess skin and reshaping of the entire breast. Fortunately, these scars are easily hidden in bras and bikini tops.

Extensive Sagging: Inverted T or “Anchor” Incision
Dramatic results for reshaping the breasts are available with the Inverted T incision. Getting its nickname as the “Anchor” lift because of the incision pattern, this technique involves 3 scars. One is around the outer edge of the areola, then another is down vertically from the bottom outer edge of the areola to the breast crease, and the last one is hidden in the horizontal crease of the inframammary fold. This technique allows for the removal of a significant amount of excess skin and sagging tissue. It is also used when the Breast Lift is combined with a Breast Reduction. This gives beautiful correction for reshaping pendulous breasts, which is when the nipple is below the edge of the crease of the inframammary fold and it is pointing downward.

Moisture – for Healthy Winterized Facial/Body Skin

It’s About Moisture…

Cold, dry winter weather can really cause irritation, discomfort and… can age the skin if proper care isn’t taken. You invest in your skin with cosmetic surgery and non invasive procedures, so it is important to take extra care of your skin…especially when the weather is extreme.  Here are some age defying tips to help you keep your skin healthy and feeling great in all kinds of weather!

Conserve Moisture
There are many habits that can strip out the moisture from your skin. This dryness can be painful, itchy and contributes to skin-aging by stripping moisture. So – work on replenishing this lost moisture using a few simple steps:

First…some habits to avoid – and some to do:
….Hot showers or baths removes the natural lubrication and oil on the skin. Although a hot shower always feels good after coming in from the cold, try to keep it short and not too hot.
…..Cleansers and toners, containing harsh ingredients, strip your skin of natural oil. Real sudsy cleansers usually have a surfactant like sodium laurel sulfate, and the acetone in many toners can be too harsh, along with already dry conditions.

…..Use mild cream or gel cleansers and toner with no alcohol this time of year.
Sipping hot coffee and tea is so comforting when it’s cold out, but beware the dehydrating effects they can have. Most people only think to drink lots of water when they’re sweating and hot, but cold, windy air can be just as dehydrating for your skin. So, drink lots of pure water everyday!

Moisture Reinforcement
Not only do you need to be on the defensive when it comes to protecting your skin (during the winter) by not participating in certain drying habits, but you need to be on the offense to add the extra help it needs this time of year. Here are some ways to add moisture reinforcement for your skin:

…..Running the heat in a home strips the air of any moisture -counter this by adding a little water vapor back to your home’s air with a humidifier.
…..Diligently use moisturizer morning and night… your face and hands are most exposed to the elements, but don’t neglect the rest of your body.
…..Exfoliate gently to remove the dull dead skin build up … then you must moisturize that fresh new skin to keep it healthy and glowing.
…..Continue using sunscreen. Snow can actually reflect up to 80 percent UV rays. As skiers and snow sports enthusiasts know, you can get a sunburn in the winter.
…..Eat foods that encourage your skins moisture retention... rich in essential fatty acids like salmon and other “healthy fats” found in avocados and coconut oil.

Refresh Your Under Eyes with Fillers

When you look at yourself in the mirror do you see dark circles, deep hollows, and tired looking eyes? Also, as fat gets displaced from behind the eye, or possibly increases and bulges under the eye, this can accentuate the aging of the eye area.

– cutting down on salty foods
– minimizing your exposure to environmental pollutants
– not rubbing your eyes so much

While these tips can prevent the tear troughs and under eye bags from getting worse, you can also reverse their appearance with hyaluronic acid fillers.

Facelift for Different Decades

Last week we determined that this is the perfect time of the year to get a Facelift. The end of summer and beginning of fall season offers the perfect downtime before Thanksgiving and winter festivities. Now, I will address the different seasons of life, men and women experience, and the rejuvenation benefits  from a customized Facelift.

Variations exist based on gender, genetics, environment and lifestyle choices… such as smoking and how diligently someone has cared for their skin. However – generally our skin reflects the decades in similar ways. With this in mind, remember that the most important factor isn’t specifically your age, but whether the face reveals the changes from aging that can be corrected by a Facelift.

Facelift In Your 40s’s

  • The signs of aging start to creep in for most people in their late 30s and early 40s.
  • These signs of aging are when the lines turn into wrinkles and
  • skin starts to lose some of its elasticity and glow.
  • Visible crows feet, frown lines, smile parenthesis  show up
  • and volume loss in the cheeks and lips become a sign of this age.

Bikini Ready with Tummy Tuck

Put Your Best Face Forward Now…

Injectable Fillers are the number one cosmetic procedure performed in the US today. You may see them advertised as Liquid FaceLifts, Fillers FaceLifts or a title coined by a surgeon including his name. No matter, they all have a few things in common:
– they are non surgical

SCULPTRA – was formulated specifically as a treatment option for facial lipoatrophy. Lipoatrophy is a condition that causes sunken cheeks, indentations and hollow eyes. Sculptra is poly-L-lactic acid and does not migrate or causes damage to surrounding tissues. Visible results from Sculptra injections appear gradually over a few months and from three to five treatments are needed for these results.

AUTOLOGOUS FAT – is one of my favorite filler options. This method uses your own body fat to treat lines, wrinkles, fat loss etc. in many body areas. Because we are using your own natural body fat – the risk of reaction and/or rejection is rare. It is reabsorbed by the body over time, so repeat treatments are needed for maintenance…but over time the body rallies and self corrects to make treatments longer lasting and more effective. Labiaplasty Can Improve Your Love Life…

No surgery in plastic surgery has seen as much growth as Labiaplasty. The latest statistics from American Society of Aesthetic Plastic Surgery shows a 44% rise in this procedure. The number of surgeons providing this procedure has risen from 21% to 29% of members of Esthetic Plastic Surgery Society. This represents an 8% increase.

Expert Study Reports Increased Sensation
*Great information in a recent paper entitled *”Prospective Evaluation of Female External Genitalia Sensitivity to Pressure following Labia Minora Reduction and Clitoral Hood Reduction.” by Dr Otto Placik has helped dispel concerns regarding post operative labial sensitivity.  His article, published in the Oxford Journals,  titled “Prospective Evaluation of Feminine Genital Sensitivity and Pressure following Labia Minora reduction and Clitoral Hood reduction” shows that rather than reducing sensitivity with surgery, there was a net increase in sensitivity to touch. It is suggested the removal of the droopy excess labia removes *in *un of labia, which may have less feeling.

More Frequent and Satisfying Sex
In addition to labial sensitivity increase, the clitoral hood when raised may have enhanced feeling. What does all this mean, well, a happier patient. In self reporting from the same group, *44 to 1 of the women had an increase in sexual relations and when they did, it was better. These labiaplasty patients reported an improvement in orgasm frequency in *35 to 3 subjects and increase of strength of orgasm *35 to 3. This information reported in the Oxford Journals for the increase shown in 2012-2013.  So in conclusion, not only did labiaplasty maintain or improve sensitivity, it made for a more frequent, better sex life.

4 Tips for Looking Great in Photos

Everyone takes photos to capture holiday memories…so this is the time to put your best face forward and be “picture posing saavy” for socialite magazines and social media like Facebook and Instagram.  We are seen by hundreds, possibly thousands, of people on Social media venues.  To help you with this…a few tips from photography experts, as well as some of my own advice on how to look great in photos.

Be a picture posing pro… Tips to help you look great in photos:

A Perfect Pout… along with bright pearly whites you must have shiny lips or your smile won’t look as good as it could.  Also, thin lips can add years to perceived age and distort facial balance and harmony. – Pose in a natural, slimming position by resting your weight on one leg. Rest your hand on the hip with shoulder and elbow pointed back or allow your hands to fall naturally at your side while slightly lifting your elbow back and away from your body. This prevents the illusion of a “fat arm” that happens when the arm is pressed against the body. This will prevent your hands from looking larger than normal.

Scarf Season…Refined Chin…The Perfect Time for Kybella

Breast Augmentation with Fat Grafting

As an expert in Fat Grafting (also known as Fat Transfer), I’ve seen how wonderful this completely native material…from your own body can naturally enhance features without the use of foreign substances. It does require great skill to be used correctly and does have some limitations. But for my natural minded patients, this could be a great option for facial volume loss and the Brazilian Butt Lift, as I’ve previously written about. Depending on my patient’s expectations it can even be an option for breast augmentation.

Managing Expectations of Fat Grafting
I like to have an extensive discussion of my patient’s expectations and their options with any procedure, but when it comes to breast augmentation, there are quite a few options to discuss. These discussions include what size breast would be harmonious for the body, the kind of implant, the type of incision used to insert the implant or whether to even use an implant. Often, fat grafting isn’t considered by my patients because it isn’t as widely publicized as implants, but I do believe that it can be a great option for many.Here are some of the reasons to consider Fat Grafting for your Breast Augmentation:
… It’s a completely natural substance coming from your own body.
… Because it’s from your own body, there is less chance of your body rejecting it.

Monsplasty – for the V Zone

Pubic Lift and “V Zone” Rejuvenation
Women no longer need to live with the discomfort and debilitation of their self esteem due to a less than satisfactory genital area. I offer a high level of sensitivity to the nature of this procedure, as well as the surgical experience to achieve desired Pubic and “V Zone” rejuvenation.
One of the fastest growing surgeries for women is the Pubic Lift, or Monsplasty, to smooth out the Mons Pubis area. The Mons Pubis often bulges with extra skin and fat after weight gain, childbirth or normal aging. What is the Mons Pubis? This is a protective padded mound over the pubic bone, which segments to create outer labia. The fatty tissue typically develops with the onset of puberty, but usually increases in size due to weight gain and childbirth. What about the “V Zone”? Another surgery to rejuvenate a woman’s genitalia is Labiaplasty. Like Monsplasty, this surgery can be performed for both cosmetic and functional reasons. The labia are affected by childbirth, weight fluctuation, genetics and ageing. Elongated or large, irregular labia can cause discomfort, painful sexual relations and traps extra moisture that can lead to yeast infections.
They can also be a point of embarrassment, increased self consciousness and poor self esteem.
